Suspension or Revocation of Teacher's Certificate.

-It is my unpleasant duty to inform you that certain charges have been preferred against you, on apparently sufficient grounds, alleging that (state the charges plainly and briefly-see Section 48, Paragraph 13th,) in consequence of which your certificate to teach a public school is hereby declared suspended (or revoked, as the case may be,)

and the right to teach a public school in this State, as well as the privileges conferred by said certificate, are suspended (or revoked, as the case may require), until further notice.

The case will be presented to the Board of Public Instruction, (or State Superintendent of Public Instruction, if the certificate had been issued by him; also state the time and place at which a hearing will be granted), at which time you shall have an opportunity to make a full and fair vindication of the charges, in conformity with the regulations of the Department of Public Instruction.
